Ascendo Resources is currently seeking a Care Coordinator for a full-time, on-site role based in Akron, OH.
Salary range is $60,000 - $65,000 (hourly), Monday-Friday, 8:30am to 5:00pm.
Our client is a privately owned, start-up home care agency based in Akron. We are seeking a Care Coordinator to join its growing operations team. This is a hands-on role with direct access to ownership and leadership.
Position Summary
The Care Coordinator oversees the coordination and quality assurance of non-medical home care services, providing direct supervision and support to caregivers, managing staffing and scheduling, and serving as the primary liaison between clients, caregivers, and office staff.
Essential Duties and Responsibilities
Caregiver Coordination & Support
- Supervise, coach, and support home health aides/caregivers to ensure compliance with company policies and care standards, including training completion
- Address performance issues and participate in corrective action/disciplinary processes as needed
- Serve as primary point of contact for caregivers on assignments, concerns, and scheduling
Scheduling & Staffing
- Coordinate and maintain caregiver schedules to ensure timely coverage of client service hours
- Match caregivers to clients based on skills, availability, and preferences
- Manage call-offs, last-minute coverage needs, and schedule changes
- Collaborate with recruiting staff on staffing needs and onboarding priorities
Client Coordination & Quality Assurance
- Participate in client intake and ongoing service coordination
- Conduct client check-ins and follow-ups; respond to complaints or service issues
- Ensure care plans are followed and services delivered as authorized
Documentation & Compliance
- Maintain accurate documentation of supervisory visits, evaluations, and client communications
- Ensure compliance with state regulations, agency policies, and payer requirements
- Assist with audits, surveys, and quality assurance initiatives
- Utilize agency software systems (scheduling, documentation, EVV platforms)
Communication & Collaboration
- Communicate regularly with administrative staff, nurses (if applicable), and leadership
- Participate in team meetings and contribute to process improvement
Required Qualifications
- High school diploma or equivalent
- 1 years of experience in home care or healthcare operations; caregiver supervision preferred
- Knowledge of non-medical home care services and caregiver roles
- Strong organizational, time-management, and problem-solving skills
- Excellent verbal and written communication skills
- Proficiency with computer systems and scheduling software
- Reliable transportation; ability to travel to client homes as needed
- Stable work history preferred
Preferred Qualifications
- Associate's or Bachelor's degree in healthcare administration, social services, or related field
- Previous supervisory/management experience in home care
- Familiarity with state home care regulations and EVV requirements
- Experience with Kantime EMR
