The Community Care Coordinator will work directly with community members to understand their needs, educate them available resources, and get them set up with a consultant to help them navigate service systems. Key Responsibilities:
- Meet with individuals to understand their circumstances, strengths, and goals across social, medical, and behavioral domains.
- Educate individuals on the process of getting services started, and the consultation process.
- Collaborate with local agencies, nonprofits, and care teams to ensure coordinated support.
- Assist Care Coordinators with documentation and administrative needs to support case management and client progress in case management software,
- Join internal and external team meetings to discuss cases, align resources, and enhance service plans.
- Provide educational materials and guidance to clients and families regarding available supports.
- Help identify community resource gaps and contribute to improvement initiatives.
- Bachelor’s degree in Social Work, Human Services, Public Health, Psychology, or a related field.
- Strong communication and relationship-building abilities, especially with vulnerable populations.
- Ability to manage multiple cases, stay organized, and follow through on details.
- Demonstrated commitment to supporting underserved communities.
- Experience with social service programs or interest in learning how to navigate them.
- Comfortable using computers; experience with documentation systems is a plus.
- Empathy, cultural sensitivity, and a client-centered approach to care.
