Youth Shelters & Family Services is seeking a compassionate and dedicated Service Coordinator to join our Transitional Living Program team, empowering individuals on their journey to self-sufficiency and independence.
What would my job responsibilities be?
- Customarily and regularly exercise discretion and independent judgment, assume responsibility and work under minimum supervision.
- Provide case management services in compliance with policies, procedures, contracts and other requirements, oversight of client services and other program services and systems.
- Maintain dynamic interaction with the Program Director concerning legal issues and program functioning.
- Provide case management services from screening and intake to discharge including completing client assessments; developing self-sufficiency plans, implementing, coordinating and reviewing; and providing discharge and after-care planning and implementation for an assigned case load.
- Assist in providing individualized life skills education, leadership opportunities and activities to facilitate achievement of client personal goals.
- Complete weekly billing reports.
- Maintain comprehensive client case files, as determined by program requirements, providing oversight of the file maintenance procedure.
- Provide information and data for required reports and contracts.
- Coordinate external relations as related to case management and other services.
- Participate in the interviewing, training and oversight of staff including volunteers and interns, as assigned.
- Engage in daily activities of the program, providing oversight of facility and grounds, maintenance, vehicle, and equipment maintenance, as well as record keeping pertaining to such.
- Participate in the emergency on-call system, as assigned.
What are the qualifications and experience I need to become a Service Coordinator?
- Demonstrated experience in providing life skills training, vocational guidance, or related services to vulnerable populations, with a particular focus on homeless youth or young adults.
- Proficiency in trauma-informed care principles, motivational interviewing techniques, and evidence-based practices for supporting youth in transition.
- Excellent communication and collaboration skills, with the ability to effectively engage with diverse individuals and groups, including youth from different cultural backgrounds, LGBTQ individuals, and individuals with disabilities.
- Preferred experience in program development, curriculum design, or workshop facilitation, enhancing the ability to create engaging and impactful programming for program participants.
- Strong knowledge of community resources and social service systems relevant to housing, education, employment, healthcare, mental health, and substance abuse treatment, facilitating effective referrals and support for program participants.
- Exceptional interpersonal skills, including empathy, active listening, conflict resolution, and the ability to set and maintain boundaries to ensure the well-being of program participants.
- Flexibility to adapt to changing priorities, work independently with minimal supervision, and collaborate effectively within a multidisciplinary team environment, contributing to the overall success and effectiveness of the program.
- Bilingual in Spanish and English.
Required
- Proficient with computer software skills including Microsoft Windows, Excel, and Word.
- Excellent oral communication skills, particularly listening, mediation, and writing skills.
- Knowledge of community resources and experienced in providing referral services to clients.
- The ability to establish and set appropriate limits with persons served to help them gain skills and confidence.
- Ability to adapt to shifting priorities.
- Ability to relate in cross-cultural environments.
- Must be able to drive company vehicles.
- Ability to organize team roles and evaluating employee performance.
- Exceptional attention to detail and time management skills.
- CPR/ First Aid Certified (will train).
- Valid Driver’s License, insurable under YSFS automobile policy.
Must be over 21 years of age to meet minimum eligibility requirements of our automobile insurance carrier.
Preferred
- Bilingual in Spanish and English.
What are the minimum education requirements?
- Master’s Degree in counseling, social work, psychology, or related field preferred.
OR bachelor’s degree in human services preferred
OR a two-year degree in Human related services and at least one-year related experience
OR a high school diploma or GED and at least two years related experience.
- Experience with homeless and at-risk youth, preferred.
- Case management experience required
