We are currently seeking a dedicated and compassionate individual to join our team as a Transitional Living & Pregnant and Parenting Youth Service Specialist. In this role, you will provide essential service coordination for young parents enrolled in our Pregnant and Parenting Youth Initiative (PPYI) overlay program and provide support to our transitional living program.

What would my job responsibilities be?

• Build trusting relationships with clients by responding to their basic needs.
• You will provide information and referral assistance, connecting participants with appropriate social service agencies and community programs.
• Assist youth by drafting referrals and offering resource materials.
• Maintain comprehensive participant case files, both in digital and hardcopy formats, in compliance with program and funding requirements.
• Collect and report program data using social service software.
• Assist in completing monthly and annual report forms, with training provided.
• Make sure to adhere to all funding requirements regarding service provision and fulfill the training responsibilities.
• Coordinate and refer participants for pre/post-natal care, well-baby care, and appropriate childcare. Provide basic support services in collaboration with YSFS' staff and community partners.
• Coordinate and deliver on-site education and individual mentoring, in partnership with community collaborators and YSFS' staff, addressing topics such as pregnancy prevention, parenting, nurturing and attachment, child development, and child abuse prevention.

What are the qualifications and experience I need to become a Pregnant and Parenting Youth Initiative & Transitional Living Specialist Support worker?

• Requires knowledge and belief in “Housing First” philosophy and strategies.
• High integrity when dealing with a broad array of cultures and restricted and/or confidential information.
• Sensitivity to cultural and socioeconomic characteristics of population served.
• A commitment to empowering others to solve their own problems.
• A conviction about the capacity of people to grow and change.
• Case management experience required.
• Bi-lingual in Spanish and English preferred.
• Experience with providing services to the homeless and low-income population.
• The candidate must be able to develop strong working relationships with client population, relate to individuals making significant life decisions, and be supportive of individuals’ increasing independence.
• Computer software skills including Microsoft Windows, Excel, and Word.
• Excellent communication skills, particularly listening, mediation, and writing skills.
• Initiative, creativity, reliability, flexibility, thoroughness.

What are the minimum education requirements?

• Master’s Degree in counseling, social work, psychology, or related field.
• OR Bachelor Degree in Human Services.
• OR a two years degree in Human related services and at least one year related experience.
• OR a high school diploma or GED and at least two years related experience.
