Schedule: Monday-Friday, hours vary/Hybrid/Full Time

Position Overview:

Under the direction of the Program Manager, the Family Team Facilitator collaborates with youth, families, and informal and formal supports, to identify individualized needs for youth. The Family Team Facilitator works in partnership with families to identify individuals who will participate in the family team, facilitates meetings with the identified parties utilizing a Wraparound approach, and holds the team accountable for accomplishing their goals. Facilitators should display effective interpersonal skills, dedication to helping youth and families reach their goals, and strong attention to detail in the coordination of services and care.

Qualifications:

• Master's degree preferred or Bachelor's in Counseling, Social Work or related field
• Experience in a counseling setting working with adolescents
• A valid driver’s license
• Reliable transportation (proof of insurance is required)
