ShiftDescription:Great opportunity for a Licensed Social Worker (part time) to work in an organization that focuses on treating the whole person, physically, emotionally and spiritually.We are seeking a Licensed Social Worker who is dedicated to providing exceptional care to those we serve at Loyola Medicine. Assesses the patient's mental and social functioning by providing a psychosocial screen/history, assessment, resource information, counseling and a discharge plan. Responds to immediate requests and ensures a comprehensive medical plan by communicating the psychosocial and/or discharge information to the medical team verbally and through documentation in the patient's medical record. Completes discharge plan requirements, i.e., appropriate transport forms, authorization for release of medical information, contacting appropriate family or agency representatives, etc. Attends service line multi-disciplinary rounds, unit meetings and meetings specific to the assignment responsibility to facilitate continuity of patient care. Provides educational and psychosocial programs to patients and families and enhances the educational programs of LUHS. Adheres to the Social Work Code of Ethics and ensures on-going professional education to provide state of the art psychosocial and discharge care. Works with Social Work students as a Field Instructor in conjunction with the Department of Social Work Student Coordinator and the specific University Liasion
Meets the Department of Social Work management accountabilities of attendance at Social Work Staff meetings, completes medical record chart notes and statistical reports, completes LUMC mandatory education classes and regulatory compliance.
