Job Overview:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and assistance to individuals and families in need. Your role will involve assessing client needs, developing care plans, and coordinating services to ensure the well-being and safety of our clients.
Duties:
- Conduct comprehensive assessments of client needs, including physical, emotional, and social factors
- Develop individualized care plans based on assessment findings
- Coordinate and monitor services provided to clients, ensuring that they are receiving appropriate care and support
- Provide crisis intervention and support during emergency situations
- Collaborate with other professionals, such as social workers, counselors, and healthcare providers, to ensure holistic care for clients
- Advocate for clients' rights and access to necessary resources
- Maintain accurate and up-to-date case files and documentation
- Monitor client progress and evaluate the effectiveness of interventions
- Provide addiction counseling, crisis management, and behavioral health support as needed
Skills:
- Strong knowledge of addiction counseling, crisis intervention, and behavioral health principles
- Familiarity with Medicare regulations and child welfare systems
- Excellent communication skills, both written and verbal
- Ability to work effectively with individuals from diverse backgrounds
- Strong organizational skills with the ability to prioritize tasks and manage time effectively
- Proficiency in case management software or similar tools
- Experience in childcare or mentoring programs is a plus
- Knowledge of Section 8 housing programs is an asset
