As part of a multidisciplinary team, initiates and engages formally homeless disabled individuals living at Permanent Supportive Housing by providing an integrated service delivery model. Develops goals and objectives for clients to maintain housing and meet their individually identify goals including but not limited to treatment of mental health symptoms, alcohol and other drug abuse issues, health care linkage, increasing income, and increasing self-sufficiency and self-determination. The services are designed to be pro-active and incorporate a philosophy of prevention services and focus on strategies to help clients change aspects of their behavior, in regard to physical and mental health and addiction. Completes and maintains progress notes and documentation according to agency and regulatory service standards.
This is a non-exempt, union position with a minimum-starting wage of $20.00 per hour.
The hours of work are 8:00am – 4:30pm, Monday through Friday.
Qualified candidates must possess a Bachelor’s degree in social, behavioral or human services, coupled with at least one year of professionally supervised work in the delivery of mental health, chemical dependency or social services. Valid driver’s license with less than six points, proof of insurance along with operational vehicle, required.
