Title: RN Behavioral Health Case Manager - Home Health
Status: 6-Month to 12-Month Contract
Location: Cuyahoga County, Ohio
Travel: Mileage Reimbursement at Standard IRS Rate
Visa Sponsorship: Not supported
Positions Available: 1 part-time & 1 full-time
The Behavioral Health RN's primary role is to function independently in providing comprehensive care to patients who are experiencing mental illness - both acute and chronic disease states.
Job Qualifications:
Experience
- 1 years of experience with Behavioral Health, Mental Health, and/or Psych Patients within the last 3 years
Education
- Associate's Degree in Nursing or higher
License, Certification, Registration
- State of Ohio Registered Nurse license
- BLS Certification
- Valid Driver's License
Additional Qualifications:
- Experience working as part of a team and working effectively with persons and communities from diverse cultural, social, and ethnic backgrounds.
- Ability to work in a complex institutional structure requiring significant collaboration.
- Strong planning and organizational skills.
- Demonstrated ability to work well under pressure, within strict time parameters, and with strong attention to detail.
- Ability to work independently with minimal supervision.
- Demonstrated ability to maintain strict confidentiality.
- Bilingual a plus
