Your Choice Caregivers LLC is a Private Home Care Agency Providing Medicaid-approved, skilled and unskilled nursing care through the Georgia Pediatric Program (GAPP), CCSP. Source. We are looking for Registered Nurse to join our team and be in charge of ADMINISTRATIVE TASKS AND SUPERVISORY VISITS.

Essential Duties:

• To maintain and update all client's files. Make sure all documents are up to date, current, signed and filed electronically.
• Prepare paperwork for new clients and current clients, ensuring all information is correct signed and filed.
• Ensure all clients have their blue folder/client folder in their home and needed supplies.
• Schedule new client intakes when needed.
• Preparing progress notes and supervisory visit entries, documenting patient's current status
• Assures payer documentation is completed properly and timely, as required.
• Enter infections and incidents/occurrences into the online Risk Management Incident Reporting System, as specified by policy.
• Provides direction and leadership to clinical team members in collaboration with the clinical director.
• Assists clinicians in coordinating the transfer and discharge of patients from agency services as indicated by the physician.
• Provides direct patient care, as necessary, in accordance to scope of practice and physician orders.
• Manage and document phone calls from physicians, clinicians, patients, referral sources, and communicates patient updates/new orders to clinicians. Uses coordination notes to document, as needed and appropriate.
• Review assessments and plans of care, per assigned workflow, and consults clinicians with recommendations, as appropriate.
• Oversees and assures development, implementation, and updates to the individualized patient plan of care, as appropriate.

Job Types: Full-time, Part-time

Pay: $65,000.00 - $75,000.00 per year
