Youable equips team members with the skills and tools to be advocates for the mission of empowering people of all ages to be emotionally healthy.
The Case Management team arranges and facilitates a variety of services for children with significant mental health symptoms that impact their life of the families served. This client-centered role works in the community to understand client perspectives, and is part of a strong team set up to support and encourage each other.
Job Requirements:
Team members manage an average of 15 active clients per month and ensure the case management treatment plans and functional assessments are completed in accordance with organizational, DHS, insurance, and county guidelines. Successful Case Managers make a qualitative difference in the lives of the families with whom they work.
The goal in the position is to improve the overall functioning of families served and reduce the need for and reliance upon out of home placements and other services that are more intensive, restrictive, and costly. Opportunities to learn and grow in this role start at day one!
