YMCA of the Triangle is Hiring a Community Case Manager-Full Time- Alexander Family YMCA Near Raleigh, NC

POSITION SUMMARY:

Under the supervision of the Director of Case Management, the Community Case Manager is responsible for providing a case management program to families within the Y Excel programs. They will also serve as a role model by exemplifying high standards and values. They are responsible for managing their functions in accordance with the stated mission, goals and policies established by the administrative staff, governing committees, and Board of Directors of the YMCA

ESSENTIAL FUNCTIONS:

Case Management

  • Identifies and maintains a caseload of at least 15-20 families that need additional support to thrive as a family. Most of the caseload will be secured through Y Excel programs. Case management services can also be provided to additional families identified at the discretion of the branch leadership.
  • Develops individualized goals with the family, educates them in goal topics, and refers them to community resources within the timeframe of 6 months – 1 year.
  • Connects the family to additional community resources including but not limited to financial support, school personnel, immigration services, food pantries, mental health services, physical health services, etc.
  • Creates transition plans for the case management family once they complete the program.
  • Develops and maintains positive relationships with community providers, volunteers, clients, program participants, and colleagues at all levels of the organization.
  • Maintains proper records; prepares required reports as directed, as well as any assessments, intake or exit interviews, data input, record keeping, or reporting required by funders and relevant state or other regulatory authorities.
  • Stays informed and well trained on various topics related to their field of work
    Community and YMCA partnerships
  • Works directly with YMCA Excel staff and other relevant YMCA departments to create a seamless network of support for a case management family within the YMCA.
  • Creates additional programming in conjunction with YMCA staff and other nonprofits to help support families.
  • Regularly connects with community partners to communicate the network of services available to families.
  • Participates in community meetings to stay connected to relevant offerings.
  • Works with the school administration to connect parents to their child’s school.
  • Included other duties as assigned by the branch staff; could include in branch LET shifts, branch meetings, and Annual Campaign responsibilities.
  • Group and Additional Community Support
    • ? Leads parent groups that meet on a regular basis around shared topics of interest
    • ? Invites guest speakers to educate parents on different topics
    • ? Encourages and trains parent leaders to help grow them in their leadership abilities
  • Provides additional support to families not receiving full case management services but need quick help accessing a nonprofit, resource, donation, and/or information.

QUALIFICATIONS:

  • Bachelor’s degree in related field preferred (social work, psychology, sociology, human services) and/or 1-2 years of related experience.
  • Bilingual in Spanish is preferred but not required
  • This person must be at least 21 years of age and must be a positive role model for children
  • Experienced in and passionate about creating and fostering communities through programs
  • Ability to think strategically and creatively about finding solutions to family needs
  • Flexibility in work schedule which will include some evening and weekends as needed
  • Comfortable traveling and meeting with families inside their homes
  • Exceptional oral, public speaking and written communication skills
  • This position requires an understanding of family systems, working collaboratively, and a culturally competent case management model

While performing the duties of this job, the employee is regularly required to use a computer for extended periods of time and be able to communicate using a computer and phone/smart device.

  • The employee frequently is required to sit and reach and must be able to move around the work environment.
  • The employee must occasionally lift and/or move up to 40 pounds.
  • Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust.
  • The noise level in the work environment is usually moderate.
  • Ability to obtain upon hire certifications in Basic Life Support, Basic First Aid, Emergency Oxygen and Emergency Medication Administration Training. All certifications must stay current while employed with the YMCA.
  • The YMCA of the Triangle requires employees to comply with all COVID related PPE, cleaning, reporting and prevention expectations efforts. Further training and detail will be provided upon question and/or hire.
