YMCA of Metropolitan Chicago is Hiring a Case Manager Housing - PT Near Chicago, IL

Description

The YMCA of Metropolitan Chicago is committed to strengthening community by connecting all people to their potential, purpose, and each other.

We are now hiring a part-time Case Manager for our Lake View YMCA- Housing site with some travel required to the Irving Park YMCA location. The Case Manager develops a comprehensive service plan individualized for the needs of each client.

Pay range for this role is $22.25 per hour, looking for help 20 hours per week during evenings and weekends

Scope of Responsibilities:

  • Work with management to interview applicants and conduct a full psychosocial assessment to determine eligibility and suitability for program enrollment; record client and program data according to requirements of the funding agency
  • Provide individualized service planning with prescribed case load of homeless disabled adults, living in permanent SRO housing. This will include complying with Life Development Center standards for recording and client file set-up
  • Provide small group leadership for client population as is appropriate under the direction of your supervisor; This will also encompass certain activities such as identifying specialized counseling issues, health/mental health needs, benefits management, legal referrals, employment support, family reunification issues, and related issues;
  • Review service plans with senior management from the Life Development Center on a monthly basis
  • Contact clients on a weekly basis to work on current supportive services; develop and implement age-specific activities in the areas of social skill acquisition, health maintenance, and appropriate nutrition needs
  • Develop and maintain a resource bank specific to the needs of formerly homeless residents, and develop professional relationships with neighborhood agencies that serve these residents
  • Assist residents in initiating and maintaining healthy family contracts, involving significant family members in the lives of residents; other duties as assigned by supervisor and/or YMCA Executive program staff
  • Inspect apartments of case management assignments on a regular basis to assure they are properly maintained, furnished, and meet safety and sanitary standards and expectations; advocate and assist participants in housing units in getting repairs when needed; visit apartments and assist Program Director in communication with landlords, particularly with repairs, cleaning, painting, and changing locks

Requirements:

This position requires a Bachelor's degree in Social Work or related degree; must have at minimum 2 years of experience working with the homeless population and/or people with challenges. Master's degree in social work or 1 year post graduate experience is highly preferred.

Child Abuse Prevention - Support the YMCA's commitment to child abuse prevention by:

  • Reporting any items that may provide a health or safety hazard to staff, members or guests to your supervisor
  • Following all policies and procedures related to keeping children safe in our facilities and programs, including those related to the monitoring and supervision of children
  • Reporting any suspicious behavior and violation of policy and procedures to your supervisor
  • Completing all child abuse prevention training as required
