Experience & Requirements

The Case Manager coordinates the plan of care for clients assigned to the Forensic Unit. Oversees patient evaluation and treatment planning, monitors client progress, adjusting treatment plans as necessary. Acts as a liaison between facility/patient/family and service delivery personnel to ensure clients are being effectively treated.

A background check, security clearance and drug screening are required for this role.

Responsibilities

Below is a list of your responsibilities as a Mental Health Case Manager with YesCare:

• Conducts patient intakes, participates in treatment team planning, and performs crisis intervention on the Competency/Restoration unit.
• May provide supportive counseling to patients and/or serve as a liaison for State and/or court services.
• Implements life skills workshops and programs in behavior management, and other psychoeducational activities.
• Maintains contact with court service agencies and health care providers involved with clients to provide information and obtain feedback on client's overall progress.
• Maintains program statistics for purposes of evaluation.
• May serve as a liaison in community resource activities.
• Participates in unit Multidisciplinary team meetings and prepares reports as needed. 
• Complies with employee standards of YesCare and applicable employee standards of the facility. Compliance includes, but is not limited to, the maintenance of a working environment and personal appearance consistent with professional responsibilities, development of harmonious working relationships, and timely notification of supervisory personnel of absences from institution.
