Job Skills / RequirementsThe Wellspring Assertive Community Treatment Case Manager provides Medicaid Targeted Case Management to homeless adults with severe mental illnesses. The ideal candidate will have a belief in recovery from mental illness and an enthusiasm for helping persons with disabilities, rooted in empathy and compassion and demonstrating a careful and caring attitude. The ACT-TCM assists clients in gaining access to medical, social, educational and other support services. The ACT-TCM develops and facilitates pursuit of individualized recovery goals via assessment, connecting to resources and monitoring progress. The Wellspring ACT-TCM's duties include but are not limited to: meeting clients in the community; working with community partners; assisting clients in applying, locating and moving into housing; supervision of clients'medications; assisting clients with their engagement in physical and mental health and addiction treatment and ensuring uninterrupted provision of all medications. The ACT-TCM models and assists clients with illness management and independent living skills and provides crisis intervention as needed. The TCM participates in monthly staff meetings, on call and collaborates daily with Wellspring clinical and program staff as well as other providers and sources of support. The ACT-TCM completes electronic documentation of services demonstrating medical necessity within 24 hours of client contact.
A personal vehicle is required as the TCM will be responsible for transporting clients to regular appointments.
