Who we are looking for:
An experienced RN Care Manager to work with patients to improve their health through coaching, referral, and connection to services and education!

As a Care Management Nurse, your tasks will include:
- Writing and documenting care plans, motivational interviewing, and building relationships with patients
- Gathering detailed patient information to identify needs, set goals, and collaborate with the care team to create individualized care plans
- Implementing care activities, maintaining regular contact with patients, and tracking plan effectiveness
- Utilizing clinical expertise to ensure compliance with medical guidelines and provide patient education
- Advocating for patients, build relationships with service providers, and provide feedback on care programs
- Participating in meetings, provide training, maintain patient files, document program activities, and adhere to healthcare regulations
Minimum Qualifications:
Education:Bachelor's Degree in Nursing or an Associate's Degree and two years of directly related RN experience
Experience
- Two years of patient-centered care
- Experience with complex patients with advanced, chronic, and behavioral health illnesses and multiple co-morbidities in an ambulatory setting
- Diverse clinical experience in working with Medicaid, Medicare, and private insurance beneficiaries with two or more chronic conditions
Certifications and Licenses
- Chronic Care Professional (CCP) Certification or the Iowa Chronic Care Consortium Certification is preferred
- Washington State RN license
- Must obtain Oregon State RN license within 180 days of hire
Skills
- Bilingual (English/Spanish) preferred. (level 10 for bilingual differential pay)
- Knowledge of chronic care management, risk stratification, and population health
- Knowledge of health utilization management, care coordination, and cost management
- Knowledge of human service programs.
- Independent worker, culturally sensitive, strong communication skills
- Detail-oriented, can handle multiple projects efficiently & meet deadlines
- Basic proficiency in Microsoft Outlook, Word, and Excel
