CASE MANAGER- RN WOUNDTECH

Summary: Assumes overall accountability in the coordination and collaboration of an assigned group of patients. This includes, but is not limited to, utilization of skills in organizing, planning, evaluating, and communicating an interdisciplinary patient care approach while adhering to evidence-based research and professional standards related to complex wound care management. In addition, the RN Case Manager establishes goals and a strategy for meeting the continuing care and discharge needs of the patient, family, care provider, and other ancillary team members while adhering to regulatory and/or payor requirements, as needed. The RN Case Manager also participates in performance improvement initiatives to foster an ongoing commitment to continuous quality assurance and positive patient outcomes.

Duties: All duties performed will be done accurately and in a timely manner.

  • Serves as the organization’s central point-of-contact for an assigned group of patients’ wound care management.
  • Assesses the patient’s wound care needs and collaborates with the wound care team in developing a plan of care.
  • Coordinates and communicates care with other health care professionals to ensure the patient receives the care and resources needed to heal the wound(s) in a timely fashion.
  • Evaluates and collaborates the need for and/or the efficiency of supplies, durable medical equipment (DME), ancillary health care services, procedures and other resources under the provisions of the patient’s applicable health benefit plan.
  • Responsible for services such as: setting up home health; ordering DME; ensuring Woundtech Field Clinician and/or other collaborating provider’s orders are completed; reviews and communicates lab and radiology results to Woundtech Field Clinician and/or other collaborating provider; communicates patient and/or caregiver(s) concerns to the team; orchestrates the sending of reports to referring physicians; and participates in data base entry.
  • Makes clinical decisions for the patient’s wound care management.
  • Implements provider’s orders and other nursing procedures.
  • Accurately documents in the patient’s medical record.
  • Performs ongoing assessment of the patient’s wound care progress.
  • Evaluates the patient’s response to care, provides input and recommendations to the clinician and/or other providers involved with the patient’s wound care management, and implements and communicates revisions to the plan, as needed.
  • Actively participates in patient chart audits, reviews and other quality assurance appraisals.
  • Helps the patient and/or caregiver(s) understand their condition and treatment options.
  • Coordinate community resources and other agency disciplines participating in patient care.
  • Oversees and participates in the patient and/or caregiver(s) education as it relates to their wound care management; this includes setting up calls (phone, webinars, etc.) with the patient and/or caregiver(s) as it relates to teaching purposes (this may occasionally occur outside of “normal” business hours to accommodate the patient and/or caregiver(s) schedule, accordingly).
  • Establishes a culture and/or sense of “urgency” to ensure the patient’s wound heals in a timely fashion.
  • Manages and coordinates weekly () clinical rounding updates/calls with the internal wound care team.
  • Manages and coordinates monthly or other scheduled/as needed clinical rounding calls with designated/external health benefit plan stakeholders.
  • Provides emotional support to patients, families/caregiver(s).
  • Creates an environment of teamwork and open communication.
  • Ensures patient and/or caregiver(s) concerns are addressed in a timely manner.
  • Participates in telehealth coordination and patient/caregiver(s) meetings, as needed.
  • Participates in organizational committees, trainings and other meetings as directed.
  • Assists in organizational decision-making as it pertains to product, supply and formulary management, as required.
  • Provides a positive working environment with all internal and external members involved in the patient’s wound care management.
  • Communicates in a clear and understandable manner using appropriate verbal and non-verbal styles.
  • Assists with special projects as needed.
  • Maintains strict confidentiality.
  • Other duties as assigned.

Education and Experience:

  • Associates Degree in Nursing (ADN) required.
  • Bachelor of Science in Nursing (BSN) preferred.
  • Three (3) or more years’ experience as a clinical RN, two (2) years of which is in an acute care setting.
  • Current RN license in the state of practice – to be maintained throughout the duration of employment in the position.
  • Wound care experience preferred.
  • Case Management certification preferred.
  • Wound care certification preferred.
  • Organized and resourceful self-starter; strong ability to work in a team.
  • Excellent computer skills – data entry, outlook, excel.
  • Strong attention to detail.
  • Excellent verbal and written communication skills.

Work Environment: Office setting in Hollywood, FL

Physical Demands: Requires manual dexterity, sitting, standing, stooping, reaching, kneeling, crouching, bending, walking, lifting up to 25 lbs. Close vision and ability to adjust focus.

Travel Requirements: 0-10%
