Eskenazi Health serves as the public hospital division of the Health & Hospital Corporation of Marion County. Physicians provide a comprehensive range of primary and specialty care services at the 333-bed hospital and outpatient facilities both on and off of the Eskenazi Health downtown campus including at a network of Eskenazi Health Center sites located throughout Indianapolis.

Job Role Summary

The Community Health Worker will work with patients at the health center and community level to enhance understanding and adherence to treatment protocols, identify social determinants of health, and help patients to set and achieve their self-management goals. The CHW will document home social and environmental assessments timely, accurately and with good quality, identifying barriers to accessing appropriate healthcare or other concerns within the patient's home environment.

Essential Functions and Responsibilities

  • Proactively contributes to Eskenazi Health's mission: Advocate, Care, Teach and Serve with special emphasis on the vulnerable population of Marion County; models Eskenazi Health's values
  • Documents health, social and environmental assessments timely, accurately and with good quality, identifying barriers to accessing appropriate healthcare or other concerns within the patient's home environment
  • Performs home visits to patients to provide support, encouragement and guidance
  • Develops supportive and therapeutic relationships with patients and their families
  • Accurately documents home/telephone visit outcomes in a timely manner
  • Utilizes motivational interviewing to encourage patients to modify their behaviors
  • Recommends community resources for patients order to meet the basic five needs of : housing, food, clothing, transportation and utilities
  • Carries a caseload of at least 100 patients
  • Advocates for patients to ensure access to needed services which includes developing community relationships and resources
  • Identifies any high risk patient symptoms or circumstances that would result in risk for self-harm or harm to others, and notifies appropriate higher level clinical staff for consultation
  • Completes appropriate home assessment documentation, identifies barriers and refers patient to the appropriate community resources
  • Works with family, significant others and community supports to assist patients in meeting goals
  • Alters activities/behaviors to reflect and ensure adequate care appropriate to the age of patients served
  • Provides quality case management and provides training to patients on activities related to daily living skills
  • Monitors patient progress and reflects this in documentation during follow up visits
  • Demonstrates commitment to professional growth by actively participating in clinical supervision

Job Requirements

  • High school diploma or equivalent
  • One year of job-related work experience, performing similar functions in a similar role
  • Valid Indiana driver's license that meets Eskenazi Health's driving policy requirements
  • Knowledge of the Marion County community and its residents having either lived in or around targeted areas; knowledge of available community resources and required application processes

  • Ability to establish and maintain effective working relationships with superiors, coworkers, patients and families
  • Willingness to work with patients and their family members or other support persons in the clinic, hospital, home, and community settings

  • Strong interpersonal skills; ability to be both clear and concise in written and verbal communication
  • Use of discretion and independent decision making regarding patient safety and care
  • Willingness and ability to carry out directives from other team members
  • Ability to use communication skills appropriate to the age of the patient
  • Ability to interpret and apply complex directives/information/regulations
  • Crisis intervention skills helpful
