Job Role Summary

Performs outreach activities to identify, remind and recall clients whose immunizations are not up-to-date, as well as provides educational information at health fairs, school events and other outreach venues as requested. This position also assists with case management for the Perinatal Hepatitis B program.

Essential Duties

  • Assesses the immunization status of clients to determine the need for follow-up and/or additional immunizations.
  • Regularly staffs the record release area providing clients with immunization records for themselves and their children. In addition, serves as a resource for clients on vaccine schedules and assesses records for clients as listed above.
  • Assesses the immunization status of children attending school, daycare, or other programs to determine if requirements are met.
  • Requests immunization information from other health care providers and parents for patients whose records are incomplete.
  • Evaluates work processes and procedures; makes recommendations for changes to enhance job performance.
  • Performs follow-up activities designed to inform parents of their child’s immunization status and encourage parents to obtain needed vaccines.
  • Provides information regarding location, dates, and times of immunization clinics.
  • Enters immunization information into INSIGHT.
  • Maintains confidentiality of all patient information according to HIPAA guidelines.
  • Assists in special immunization clinics, including but not limited to, back-to-school events, flu clinics and health fairs.
  • Provides immunization information to other health care providers, schools, and parents.
  • Prepares reports and daily logs as required.
  • Assists Nurse Epidemiologist in providing case management for infants born to mothers with Hepatitis B by ensuring that appropriate follow-up is provided for the infants in a timely manner.

Associated Job Duties

  • Assumes responsibility for self-growth by attending in-services and/or conferences, and engaging in other educational activities related to immunizations.
  • Assists in the delivery of vaccine and supplies to district health offices, other providers, and clients.
  • Participates in public health emergency preparedness exercises and in the response to public health emergencies, as directed;
  • Participates in a minimum of two H.O.P.E. events totaling a minimum of eight (8) hours annually;
  • Other duties as assigned.

Qualifications

  • Skills and knowledge acquired through completion of high school or equivalent
  • Data entry experience

Licenses/Certifications Required

  • Valid Indiana driver’s license and access to vehicle for frequent in-county travel.
  • Must meet the “Driver Requirements” as stated in the Health and Hospital Corporation Policy No. 200-1, “Vehicle/Driver Safety Program”.

Knowledge, Skills & Abilities

  • Must have knowledge of correct English grammar, punctuation, spelling and medical terminology as related to immunizations and other health care issues.
  • Must be able to communicate effectively and tactfully with physicians’ offices, school personnel, Indiana State Department of Health personnel, parents, clinic staff, and patients both orally and in writing.
  • Must be able to convey sensitivity to patients’ needs and situations.
  • Must effectively orient and instruct new employees.
  • Perform arithmetic computations, recognize, interpret, and correct errors in source documents, printouts, and immunization records.
  • Sound judgement and discretion is necessary to extract and disseminate information from records, classify types of vaccines patients have received in the past, and to establish workload priorities.
  • Strict attention to detail and concentration is required to perform data entry duties.
  • Ability to function effectively under pressure of regular deadlines and frequent interruptions.
  • Sound judgement essential in contacts with public, medical, and public health community to ensure accurate information dissemination.
  • Must make decisions as to when to involve supervisor with programmatic issues.

Working Environment

  • May be exposed to inclement weather.
  • Some contact with communicable diseases and/or infections in clinic/community settings.
  • Must be able to lift and carry up to 40 lbs.
  • Hazards such as animal bites, unsafe dwellings, difficult or hostile patients/clients.
