Winona Senior Services, Inc. is Hiring a Social Worker-Hospice Near Winona, MN

Hospice Social Worker
1.0 FTE 80 Hours a Pay Period
8 am to 4:30 pm
No Weekend/No Holidays

Position Overview: The Social Worker is responsible for direct social work services to patients and their families/significant others to help them adjust to losses which are related to illness, injury, hospitalization, or nursing home placement. As a caregiver of the interdisciplinary team, the Social Worker assists the caregiving team in understanding the significance of social, cultural, economic, emotional and family factors in relation to the patient's illness or dying process. This position may include other duties as assigned. Works closely with multi-disciplinary caregiver team in planning for patient care needs and assisting team caregivers (individually and in team meetings) in understanding the significant social and emotional factors related to patient care needs.

Essential Duties & Responsibilities:

  • Coordinates all hospice information visits, admissions and need for support services
  • Assesses the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment and communicates findings to the registered nurse and other members of the interdisciplinary group.
  • Provides an assessment in the patient's identified residence and assistance when this is not safe and another plan is required.
  • Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high-risks for suicide, neglect or abuse and plans intervention based on evaluation findings.
  • Facilitates care conferences as needed to update and educate patient, family caregivers and community resources to patient’s status and treatment needs to increase support to patient and promote continuity of care throughout course of care.
  • Collaborates with other members of the caregiver team to assure patient’s needs and desires are met with the current plan of treatment.
  • Is knowledgeable of and keeps current on available programs and resources to assist patient, family and staff in utilizing community resources. Provides information and referrals for the patient to the community.
  • Keeps updated on current Hospice Social work practice, ethics and legal issues related to patient-based services.
  • May coordinate the Hospice Interdisciplinary team meetings to assure all state and federal regulations are followed. Assures all patient care issues are discussed and resolved.
  • Will assist the Director of Bereavement in maintaining a bereavement program to meet the needs of families/caregivers for 13 months following the death of a patient to ensure compliance with all state, federal, and CHAP regulatory requirements
  • Ensures the initial bereavement assessment is completed, or assigned to appropriate team member.
  • Assists Chaplain when needed making condolence call to bereaved.
  • Assigns and oversees volunteers making bereavement calls.
  • Continually works to gain better understanding of workflow of the department and uses knowledge base to analyze department processes and makes changes to improve efficiencies.
  • Ongoing education and understanding of Continuous System Improvements (CSI) and participates and supports standard work processes and process improvement initiatives.

Essential Skills and Experience: 4-year degree in related field of study. Graduate of an accredited Social Work Program. MSW or LSW with appropriate supervision required. Valid Driver’s License. CPR Certification. Must possess or be willing to possess a MN and WI Social Work license. Word. Excel. Access or another database. PowerPoint. Outlook. Patient information database (Cerner, EMR, MatrixCare). Must be proficient in Internet searching and research.
