Position Overview:
The Population Health Coordinator is responsible for supporting practices and the population health team by facilitating the development, rollout, and maintenance of clinical programs to ensure the provision of high quality, service-orientated patient care and supportive services. Under the direction of the Associate Director of Population Health, this position will support the Care Management & Quality Programs, as well as help strengthen Ryan’s care delivery model in pursuit of clinical excellence. This work is carried out in support of the mission and goals of the Ryan Network.
