Nurse Case Manager
Salem, OR WFMC Health is a 501(c)(3) non-profit organization in Salem, OR, providing primary, specialty, and mental healthcare services to a diverse population of patients residing in Marion and Polk counties. WFMC is seeking a Nurse Case Manager to join our clinic. You will work alongside a diverse team of primary care and mental health providers including nurse practitioners and medical doctors in multiple specialties including pediatrics, family practice, and internal medicine. The clinic environment is collegial and collaborative. Patient and staff populations are multicultural and multilingual.

The Nurse Case Manager role is a full-time (M-F) role. The position will include telephone and in-person assessment of socioeconomic, medical, and behavioral health needs of complex adult and pediatric patients. The position is responsible for referrals to community agencies, care coordination, the transition of care from hospital discharges, collaboration with other members of the health care team regarding the ongoing plan of care and identify a plan for follow-up. There will also be a need to attend and participate in patient care conferences and meetings as requested, and other duties as assigned.
Qualifications:
· Current RN or LPN license in the State of Oregon
· Current CPR certification
· Previous hospital, clinic, school nurse, or other experience
· Knowledge of disease management and levels of care including inpatient, emergency, home health, hospice, SNF, ICF, ALF, rehab, and long-term care
· Understanding of outpatient clinic workflows
· Experience working with Electronic Medical Records,
· Experience working with typical office software.
Desirable characteristics
· Bilingual in Spanish preferred
· Case management experience preferred
· Familiarity with community agencies and resources
· Ability to work collaboratively with providers and clinic staff and effectively and efficiently multi-task in a fast-paced, changing environment
· Strong willingness and desire to work as a team member toward meeting an array of quality benchmarks
