Qualification/Responsibilities:

• Must possess, as a minimum, a Nursing Degree from an accredited college or university, or be a graduate of an approved Licensed Practical Nurse or Registered Nurse
• Conduct and Coordinate the development and completion and the resident assessment (MDS) in accordance with current regulations and guidelines
• Must be experienced in PDPM, MDS coding, insurance updates, care planning
• Assist in completion of patient discharge
• Auditing the resident assessment (MDS) and care plan for completeness and accuracy.
• Develop, implement and maintain a quality assurance program resident care plan.
