Job Summary:

We are seeking a dedicated Case Manager to join our team. The Case Manager will be responsible for coordinating patient care and providing support to individuals in need. This role requires a compassionate individual with a strong background in healthcare and patient advocacy.

Duties:

- Conduct assessments to determine patients' needs and develop individualized care plans
- Collaborate with healthcare providers to ensure comprehensive and timely care delivery
- Monitor patient progress and adjust care plans as needed
- Provide education and resources to patients and their families
- Advocate for patients within the healthcare system
- Maintain accurate and up-to-date patient records

Experience:

- Previous experience in a healthcare setting, preferably in nursing or behavioral health
- Knowledge of managed care principles and practices
- Strong background in patient care, including acute care and pediatrics
- Familiarity with medical terminology and HIPAA regulations
- Experience working in primary care, hospitals, or similar healthcare environments
