Westminster Canterbury Richmond is Hiring a Home Health and Hospice Aide Near Richmond, VA

As the Home Health & hospice Aide you will provide personal services such as bathing, grooming and assistance with ambulation to skilled home health patients under the direction and supervision of a registered nurse or therapist. Complete personal care interventions according to the plan of care and documents accurately.

JOB SUMMARY:

Assess the psychosocial status and needs of clients. Assist the home health & hospice team in recognizing and understanding the social/mental stress and disorders that impact client health and rehabilitation.

MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:• Provide Medical Social Services in accordance with a plan of care, including individual counseling, community resource planning, and crisis intervention.• Assess the client’s psychosocial status• Develop and implement a medical plan of care and revising as necessary• Assist other team members in understanding the social and emotional factors related to the health problems• Work with family as needed• Prepare clinical and progress notes• Utilize and provide client/family with appropriate community resources as needed• Participate in discharge planning• Consult and coordinate services with the primary care physician and other disciplines regarding changes in the client’s needs• Participate in in-service programs• Comply with agency policies and procedures relative to infection control and safety
