JOB SUMMARY: Assess the psychosocial status and needs of clients. Assist the home health & hospice team in recognizing and understanding the social/mental stress and disorders that impact client health and rehabilitation.
MAJORACCOUNTABILITIES/ESSENTIALJOBDUTIES:
* Provide Medical Social Services in accordance with a plan of care, including individual counseling, community resource planning, and crisis intervention.
* Assess the client's psychosocial status
* Develop and implement a medical plan of care and revising as necessary
* Assist other team members in understanding the social and emotional factors related to the health problems
* Work with family as needed
* Prepare clinical and progress notes
* Utilize and provide client/family with appropriate community resources as needed
* Participate in discharge planning
* Consult and coordinate services with the primary care physician and other disciplines regarding changes in the client's needs
* Participate in in-service programs
* Comply with agency policies and procedures relative to infection control and safety
