Job Description

Westminster-Canterbury on Chesapeake Bay is currently hiring a Full Time Resident Services Social Worker.

Our Full-Time employees are eligible for Medical, Dental and Vision Benefits on the first day of employment!

What will I do?

Our Resident Services Social Worker helps to offer social work and care management services for our Independent Living and Assisted Living residents. This person will be working with residents and families to identify and develop plans to meet their emotional and psychosocial needs.

  • Assist residents and families during transition of care process. (Ex. Admissions and Discharges from Independent Living and Assisted Living)
  • Arrange various supports to relieve psychosocial stress
  • Coordinate on-site programs to improve resident access to public resources.
  • Assess short and long-term care needs of residents with an Interdisciplinary Team of professionals to ensure proper placement within our continuum of care.

This person is responsible for protecting the rights of residents in all aspects of living, decision making, and care.

What do I need for this role?

  • Bachelor's degree in Social Services, Human Services, Gerontology, Nursing, Public Health, or a related field.
  • Master's degree in Social Work preferred.
  • Experience with Lifeplan Communities desirable.
  • Excellent communication skills and ability to mediate resident and family concerns.
  • At least two years experience working with the aging population.
  • Proficiency with Microsoft Office Products (Word, Excel, PowerPoint) and Electronic Medical Records.
