We provide evidence-based integrated treatment for individuals and families living with mental illness and co-occurring substance use disorders. Men come from all over the country for our specialized treatment and we fully integrate both family and community into every stage of the treatment process. Summary Job Description:

The Admissions Case Manager is part of the Admissions/Customer Relations Team and ensures that inquiries are responded to promptly and screened appropriately. Responsible for developing and monitoring relationships with individuals and families as well as referral sources. Supports case management responsibilities related to admission, transition and/or discharge of WestBridge participants. Participates in the development of internal as well as external customer relations. Contributes to event planning and business development activities.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:

Spends at least 50% of time managing aspects of the pre-screening & admissions process for prospective participants:

  • Responds to admissions calls in a timely manner.
  • Conducts initial phone engagement and screening for individuals to all programs.
  • Primary contact for completion of intake screening and admissions coordination for the Intensive Outpatient Program (IOP).
  • Supports intake screening and admissions processing for residential and Community Integration Program (CIP), transitioning prospective participants to the Admissions Clinician(s) for further clinical interview.
  • Case management responsibilities related to admissions, transitions, and discharges
  • Assists in the identification of aftercare programs or resources for participants transitioning out of all programs WestBridge.
  • Develops and maintains relationships with other providers and referral sources, as it relates to admissions and discharge case management duties.
  • Supports admissions, transitions and discharge case management needs for IOP participants (possibly meeting with participants and their family members for tasks related to insurance benefits, housing, community resources, etc.).
  • Completes quality assurance duties as needed.
  • Responds to and prepares medical record requests.
  • Conducts chart audit process for IOP admissions.
  • Maintains documentation as it relates to admissions and case management duties.
  • Attends and participates actively in organizational committees, representing the admissions team.
  • Attends and actively participates in both individual and team supervision on a weekly basis
  • Demonstrates customer service skills: General friendliness, a solution-focus attitude, being responsive and flexible. Able to remain objective and not personalize situations.
  • Models a positive, hopeful attitude to the support the WestBridge culture.
  • Plans and participates in event planning for annual Family Weekend of Hope as well as other staff special events related to customer relations.
  • Performs other duties as assigned.

Requirements

  • Knowledge obtained through completion of a Bachelor’s Degree in appropriate discipline (e.g. Human Services, Mental Health, Social Work) from accredited institution.
  • Skills in admissions screening and/or participant assessment, including the use and withdrawal from substances, change in psychiatric status, physical health and safety. Familiarity with the LOCUS and the VASA assessments is preferred.
  • Working knowledge in the treatment of individuals with mental health and substance use disorders, including an understanding of continuums of care.
  • Strong organizational, communication (verbal and written) and time management skills.
  • Strong computer skills with experience and knowledge of Electronic Medical Records systems (EMR) and Customer Relationship Software (CRM).
  • Demonstrates a high standard of professional conduct, professional boundaries and ethical behavior that will enhance the quality of care and encourage healthy interactions among staff.
  • Knowledge of state and federal rules and regulations governing confidentiality, HIPAA, and 42 CFR Part 2.
  • Valid drivers’ license and appropriate vehicle insurance (liability limits of at least 100,000 bodily injury per person /300,000 bodily injury per accident/100,000 property damage).
