West Valley Detox Treatment is Hiring a Case Manager for Substance Abuse Rehab Near Tarzana, CA

Summary:

The Case Manager is responsible for providing comprehensive case management and recovery focused services to residents in a residential setting. Assists with caseload clinicians, participates in treatment planning/reviews and implementation, and maintains timely, complete, and legible clinical documentation. Responsible for contributing insightful, practical, and meaningful information to the treatment planning process through running groups and individual assessments. Receives clinical supervision from Clinical Director.

Minimum Qualifications:

  • Current California Driver’s License.
  • Must be physically and mentally able to assist in an emergency.
  • Must have the ability to monitor appropriate documentation.
  • Must have a TB screen, Health Screening, CPR/First Aid certificate within 7 days of hire.
  • Must have and keep registration or certification up to date at all times to provide substance abuse recovery services.

Duties and Responsibilities:

  • Conducts Psychosocial Assessment, assessing resident’s needs and identifying presenting problems. Summarizes findings in written, integrated assessment. Demonstrates ability to develop integrated summaries relative to the clinical and psychiatric antecedents of the dependency process.
  • Develops and implements an individualized treatment plan for each resident using measurable goals and objectives.
  • Conducts groups and individual sessions with residents per program standards and documents resident progress and participation.
  • Conducts all care in a professional and therapeutic manner, placing the resident’s dignity and rights above personality differences.
  • Obtains information from the intake assessment, medical history questionnaire and physical screening, family assessment and psychosocial assessment and uses this information to formulate a treatment plan that is individualized
  • Maintains comprehensive clinical records for patients, including all services provided as part of individualized treatment plans.
  • Works collaboratively with the clinical team to engage, communicate, educate, and coordinate care with patients, families, health providers, etc.
  • Provide assistance in maintaining, monitoring, and modifying covered behavioral health services/treatment services.
  • Ensure all appropriate referrals for identified services on the treatment plan are made and coordinated.
  • Ensures documentation adheres to all regulatory time-frames and requirements.
  • Interviews residents and families, assessing need and risks to develop and implement substance abuse treatment/rehabilitation care.
  • Assists the resident in discharge planning as needed, i.e. provides referrals when necessary.
  • Transports and accompanies residents off-site as needed and required and at the direction of Clinical Director.
  • Attends all staffing, in-service trainings and meetings as required, including weekly clinical staffing and team meetings, as well as any additional meetings as requested by the supervisor.
  • Additional responsibilities may be assigned to the case manager as needed.
