WEST SIDE COMMUNITY HOUSE is Hiring a Community Health Worker Near Cleveland, OH

MomsFirst provides support to expectant Teen mothers & families living in the City of Cleveland through the four Healthy Start Core Services of outreach, case management, interconceptional care, health education, screening and data collection. WSCH will engage Teen-Aged Expectant Moms

Work Hours: 40 hours per week, some evenings and weekends, flexibility of schedule required.

Duties:

· Responsible for the provision of service to all of the clients enrolled in MomsFirst at WSCH to also include group education facilitation within CMSD Schools .

· Engage and coordinate with CMSD, local hospitals and medical centers to recruit participants.

· Enroll and maintain pregnant students from assigned 9th-12th grade schools in the Cleveland Municipal District, including charter and vocational schools.

· Maintain all client files in paper and database form.

· Provide clients referrals on client behalf for community resources.

· Complete all training as required by the model of services to include the Ages and Stages Questionnaire (ASQ).

Qualifications:

· High School Diploma with 2-3 years of relevant experience in case management, home visiting, outreach, and social services.

· Community Health Worker Certification preferred. Will train if qualified.

· Excellent written, organizational, time management, verbal & communication skills.

· Proficient in Microsoft Office and ability to work on multiple projects concurrently.

· Ability and desire to work with diverse populations.

· Must have a valid Ohio Driver’s License and an insured vehicle which can be used for work purposes and be able to pass a criminal background and drug test.
