*Community health workers collaborate with healthcare providers in assisting patients with education and support to improve healthcare outcomes.

*They provide intensive case management to patients and assist with access to care, barriers in obtaining necessary resources; both community and insurance based.

*In addition, they will act as a guide in navigating transitions of care during a patients health care journey.

*The location for this position is the Cleveland Medical Center, Main Campus.

*This position is full time 40 hours per week, days.

*The population will be Emergency Services.
