Wesley Community Health Services is Hiring a Wellness Nurse (RN) Near Kent, WA

  • Employees, and their families if elected, can participate in medical and vision insurance (full time and ACA eligible), dental (full time) and group life (employee only for full time, excludes part-time and on-call staff).
  • We offer a combined paid time off (PTO) policy which incorporates state paid sick leave with company paid time off at an accrual rate of 0.0607 per hour worked, equivalent to 120 hours of PTO at 2,080 hours worked in a year. We also offer 6 Paid Holidays (8 hours for full time and 6 hours for part time per event, excludes on-call staff) and 1 Personal Holiday of Choice per year (excludes on-call staff).
  • Retirement planning is encouraged through our 403(b) plan that includes a generous 100% company match on the first 4% of earnings an employee contributes. There is a 5-year vesting schedule on the company match, and minors are ineligible for the company match.
  • A food and beverage discount of 50% is available to all employees at any Wesley bistro. Employees are also eligible for On-Demand Pay with Dayforce Wallet. Minors need parental consent to access this benefit.

Enrich thelives of older adults through community, choice, and continuing care in the RNWellness Nurse role. The Wellness Nurse promotes safety and wellbeing, supportstransitions in care level, and provides medically-related services toindependent residents throughout the retirement community. This is accomplished through nursingassessments, early intervention, and ongoing health education. Additionally,the Wellness Nurse plays a key role in educating residents about the full scopeof care services available throughout Wesley. Find your sense of belonging at Wesley!


 You willprovide continuing care through these responsibilities

Essentialfunctions of this position include the following.

  • Meet with new residents and present the Notice of Servicesdisclosure.
  • Serves as the primary point of contact for nursing questions andservices for independent and catered living residents within a retirementcommunity.
  • Coordinate or provide wellness services to residents such as bloodpressure checks, health education, communicable disease tracking, medicationmanagement or administration, wound care, specialized foot and nail care, dentalservices, etc.
  • Provides and support diabetic management which may include programenrollment, assessment, recertification or coordination with outside provider.
  • Make off-site assessment visits to residents moving to or returningfrom a higher level of care, in collaboration with the Home Care Manager.
  • Understands in Home Care Services regulations and strives to meetand abide by these policies and procedures.
  • Track and supports residents’ transition back to the retirementcommunity following an offsite stay or visit to a hospital or skilled nursingfacility.
  • Coordinates with hospital, skilled nursing facility, dischargeplanner or social worker to ensure appropriate care upon return home.
  • Evaluates and recommends transitions between levels of careleveraging the full suite of care services provided across Wesley.
  • Assists residents in locating external, community-based servicesas needed.
  • Responds to pendant and emergency calls on campus.
  • Participates in Resident Review meetings.
  • Conducts resident safety checks.
  • Completes incident reports in Point Click Care (PCC).
  • Provides at least one follow up resident visit for post-accidentand injury, 911 call, or hospitalization including evaluation, assessment or supportiveservices as needed.
  • Maintain accurate and up-to-date health records in Point ClickCare (PCC) or other electronic medical records system.
  • Other duties and special projects as assigned.


 Our requirements and qualifications for success

  • Bachelor’s degree in Nursing or related field.
  • Active Registered Nurse (RN) license in Washington State or MSLcompact
  • Ability to obtain a Washington State Food Worker Card within 14days of hire.
  • Ability to obtain a CPR and First Aid certification within 30 daysof hire.
  • Effective and comprehendible English verbal and writtencommunication skills to accomplish charting, record keeping, and to comprehendwritten orders and procedures.
  • Displays a personable and comforting personality to care for theresidents.
  • Use logic and reasoning to identify alternative solutions,conclusions, or approaches to problems.


 Physical, environmental,and mental requirements

  • Ability to give direct patient care, including participating inlifting, transferring, positioning, and assistance of residents with walking orin wheelchairs.
  • Able to lift 50lbs on a regular basis.
  • Ability to use a computer keyboard or touch screen to documentresident records.
  • Apply your physical and muscle strength to lift, push, pull orcarry objects.
  • Ability to utilize your lower back and abdominal muscles tosupport part of the body repeatedly and continuously over time withoutfatiguing.
  • Ability to listen to and comprehend information and ideas providedthrough conversation, email, and text.
