DescriptionHello! We are so happy you are interested in becoming a part time Admissions Coordinator with Wellspring. When you join our team as an Admissions Coordinator, you'll be part of a team of nurses, CNAs, administrative professionals and senior service support staff who are committed to supporting our staff in their work of serving seniors and their families at the Livonia Woods Campus. You'll bring an associates degree and have experience with admissions, specifically within in a long-term care, skilled nursing or geriatric nursing environment.
As an Admissions Coordinator, you will:

• Responsible for ensuring monthly and annual census goals are met/exceeded
• Authority to accept patient referrals
• Review all referrals and respond timely to referral source (typically within 15 minutes)
• Completes admission contract with patient/family within 24 hours of admission (48
  hours on weekends)
• Reviews conversion ratios and referral source trends
• Interacts with the hospital discharge planner
• Attend hospital meetings as directed
• Travels to hospital in order to conduct assessment (if needed)
• Determine organizations clinical and financial ability to meet the needs of potential
  patients/residents including high cost medication, treatment needs and equipment
  needs
• Maintain up to date knowledge as it relates to current regulations governing all payers
• Maintain accurate referral information in appropriate database, i.e., IRM and AllScripts
• Assist with completion of documents based on current policy and procedure
• Complete insurance verification and pre-certifications
• Develop good working relationships hospital discharge planners, area physicians and
  other referral sources through delivery of patient discharge packets
• Develops and implements a sales strategy that leads to the conversion of referrals to
  admission status
• Maintains a working knowledge of Medicare, Medicaid and other insurances for short and long- term care services. Consults with internal support team members as needed
• Works directly with confirmed admission for completion of admissions paperwork
• Informs the Administrator(s) of obstacles or barriers to admitting residents and
  recommends solutions to such obstacles and barriers
• Maintains knowledge of pertinent information such as anticipated discharges, potential
  admissions, hospital discharges and available beds listing related to working current referrals
• Communicates with internal admissions coworkers and external referral
  partners/sources to coordinate pending admissions
• Maintain occupancy and payer mix based on current budget
• Obtain required insurance and clinical information and submit to appropriate
  department(s)
