WELLBOUND Certified Home Health Agency is currently seeking FULL TIME Registered Nurse (RN) Case Manager to join our team to assist us with our Geriatric client, to reach their highest potential. There is 1:1 paid training with the supervisor, supportive staff, and an abundance of available resources.

Responsibilities Registered Nurse (RN):

• To mange the care of our geriatric population.
• including coordinating with the MD, field nurses, PT, OT, and ST.
• Assure quality care.
• Create Plans of care and specific to the need of the patient.
• Interact with patient or parents of the patient to ensure that we are meeting the specific needs of the patient.
• Maintaining compliance with all Federal state and local regulations.

Qualifications / Requirements Registered Nurse (RN):

• NYS License RN
