Registered Nurse - Home Health
- Provide skilled nursing care to patients in their homes
- Perform comprehensive assessments and develop individualized care plans using OASIS
- Administer medications and treatments as prescribed by physicians
- Monitor and evaluate patient progress and adjust care plans as needed
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Coordinate with other healthcare professionals to ensure continuity of care
- Assist with discharge planning and transition of care from hospital to home
- Provide health coaching and support for patients with chronic conditions
- Perform specimen collection processing as required
- Minimum of 2 years of nursing experience with at least 1 year in a home health setting
- Experience with completing start of care and recertifications using OASIS
- Experience in providing care for patients across the lifespan, including infants, toddlers, and individuals with dementia
- Familiarity with electronic health records
