Job Overview:
The Medical Case Manager for the Ryan White will be responsible for providing medical case management to eligible clients.

REQUIRED EDUCATION AND EXPERIENCE:

• A currently licensed social worker (RSW, CSW, LMSW, LCSW) from a program accredited by the Council on Social Work Education, OR
• A currently licensed nurse (RN or LPN) in Louisiana; OR
• A bachelor’s (required) or master’s degree (preferred) in a human service related field which includes psychology, education, counseling, social services, sociology, family and consumer sciences, criminal justice, rehab services, child development, substance abuse, gerontology, and vocational rehabilitation; AND one (1) year of post-degree experience in direct service to HIV target population; OR
• AND one (1) year of post-degree experience in direct service to HIV target population A bachelor’s in liberal arts or general studies with a concentration of at least 16 hours in one of the fields listed in item listed above ;.

• REQUIRED KNOWLEDGE, SKILLS, ABILITIES/COMPETENCIES TYPICALLY NEEDED TO PERFORM THIS JOB SUCCESSFULLY:
• Excellent organizational skills and ability to handle multiple tasks independently.
• Proficient computer skills; excellent working knowledge of Microsoft Office software applications (excel and word).
• Excellent verbal and written communication skills.
• Working knowledge of available community resources and aptitude in locating new services when needed.
• Ability to maintain confidentiality in all work performed.
• Excellent interpersonal skills: ability to interact effectively and courteously with individuals of diverse cultural backgrounds.
• Ability to travel in the community to work with clients, if needed.

PREFERRED QUALIFICATIONS:

• Experience working with individuals living with HIV and AIDS, both youth, adolescents, and adults.
• Case Management skills to assure patient education, access to community resources and crisis management as needed.
• Experience working with community-based organizations, voluntary counseling and testing for HIV/ AIDS.
• Strong interpersonal communication skills to continue to build strong relationships with local health department and other agency representatives.
• Demonstrate ability to communicate in a culturally competent manner with ethically and sexually diverse individuals.

Job Type: Full-time

Pay: $20.41 - $24.58 per hour

Expected hours: 40 per week
