Position Summary
The ED Social Worker functions as a member of the interdisciplinary healthcare team to facilitate efficient patient movement as well as manage behavioral health emergencies and other case management issues, beginning in the Emergency Room through the continuum of care. Interfaces with internal departments as well as external departments. Responsibilities include discharge planning for complex patients in the ED to prevent inappropriate admission for patients that do not meet medical necessity criteria by procuring appropriate services and resources. The ED Social Workers also may be responsible for behavioral patients’ assessments in order to determine the need for a 5150. The ED Social Worker shares responsibility for responding to a Code Bert. In addition to performing the essential functions list below, may also be assigned other duties as required.Education:
