Position: ATTEND case manager
Location: Warren County ESC, Lebanon Ohio
 
Job Opening: The Warren County ESC is seeking candidates for an ATTEND case manager. The role of the ATTEND (Absences, Truancy, Tardies Elimination Now Docket) case manager is to perform all duties necessary to implement intervention services to youth who are chronically truant from school. The ATTEND case manager will provide intensive case management to youth and families as needed. This position requires regular interactions with Warren County Juvenile Court and school district employees.
 
 
Essential Functions: 
· Connect youth and families to intervention services as needed 
· Assist families in a strength-based team planning process of determining their needs and addressing needs through formal and non-formal services
· Consult with partnering agencies to build effective case planning 
· Assist in meditation between child, family, and community partners 
· Provide services to families in their home or other locations within the community 
· Attend all court hearings 
· Daily monitoring of school attendance 
· Assist teams in development of written individualized plans of care and to assist the team in monitoring attendance 
· Assist family and team with the development of crisis plans when necessary 
· Connect youth and families to community resources 
· Maintains accurate records and to provide reports to the court as needed
· Assist in graduated incentives that are individualized for youth and families 
· Demonstrate confidentiality, reliability, and ethical behavior 
· Facilitate development of a positive working relationship between families and those providing services and supports to meet their needs 
· Reports safety/risk management issues to immediate supervisor 
· Prepare families and teams for transition from the program 
 
 
Qualifications: Candidates need to hold a bachelor’s degree or higher in Social Work or related field. An Ohio license as a LSW or LPC is preferred. Candidate must pass a background check.
Contract: 189 days
Salary and Benefits: Salary per WCESC salary schedule and commensurate with experience
