JOB DESCRIPTION: Community Resource Marketplace Coordinator

REPORTS TO: Director of SDOH Programs

PRIMARY DUTIES: The Community Resource Marketplace Coordinator will serve as a liaison between Waco Family Medicine and current and future community partners. In so doing, they will facilitate partners’ presence on WFM’s campus to connect with patients. The Coordinator will engage potential organizations in a shared agreement as well as facilitate the schedule for on-campus time. Additionally, the Coordinator will work with the SDOH team to create patient flows so that patients can easily connect even if not on campus. Once a relationship is established, the Coordinator will be responsible for evaluating the engagement as well as whether patient needs were adequately addressed on a regular basis.

  • Develop, oversee, and evaluate daily operations of the Community Resource Marketplace
  • Supervise Marketplace staff and/or volunteers
  • Develop, deliver, and evaluate needs assessments to inform the development/maintenance of Marketplace services
  • Create a hospitable environment in the Marketplace for patients, staff, and partners
  • Develop, implement and evaluate activities to promote the Marketplace to patients, staff, and community stakeholders
  • Identify potential community partners on a regular basis
  • Develop, manage, and regularly assess community partner satisfaction and effectiveness
  • Coordinate community partner events hosted in the Marketplace
  • Communicate and collaborate with SDOH Team and internal stakeholders
  • Work with IT teams to develop and implement Marketplace referral processes, including but not limited to coordination of identification, integration, and implementation of an EMR-embedded referral software platform
  • Train WFM staff on referral process and related software platforms, as well as scope of services available in the Marketplace
  • Train clinicians in use of Z codes to facilitate referral process
  • As part of a team and in conjunction with the Population Health Director, create Marketplace goals & systems for capturing, analyzing, and reporting metrics in a HIPPA-compliant manner and in keeping with VBC aims
  • As part of a team, create vision for future marketplace growth and initiatives
  • Work with Operations to create systemwide patient access to Marketplace resources

SKILLS: The ideal candidate is passionate about serving communities that are historically underserved and is particularly skilled in managing complex projects in an organized fashion. Such a candidate is skilled in building teams, developing processes, and evaluating systems to improve efficiency and outcomes. A successful candidate will possess critical thinking skills to analyze and formulate practices that address barriers to human flourishing. Given the nature of the job, the candidate will also possess excellent verbal and written communication skills, particularly amongst many key stakeholder groups. An ideal candidate is self-motivated, resourceful, impeccably organized, and maintains a strong focus on hospitality and customer service.

EDUCATION & EXPERIENCE: While there are no specific educational degrees required, experience in serving historically underserved communities is key to success. Experience developing and maintaining partnerships with community-based organizations and experience in customer service is essential.
