Your Opportunity

We are seeking a dedicated Population Health Associate to support Vytalize’s mission of delivering high-quality, coordinated care that optimizes costs and outcomes. The Population Health Associate will work alongside the clinical and operations departments and act as a liaison with our partner practices. The right candidate will be able to build rapport with ease, have strong business acuity, have a problem-solving mentality with a bias for action and results, and be able to handle a variety of different initiatives for different individuals including Vytalize clinical staff and our partners.

The Population Health Associate will recommend and implement interventions at the practice level to improve clinical quality and operational efficiency. Collaborate with practices to facilitate redesign efforts, support care teams, introduce population health management, and support achievement towards improved financial, process, and clinical outcomes. Must be comfortable being able to present, answer questions, and run meetings in both small and larger settings. Must proactively maintain a deep understanding of the processes for each practice, be able to translate data insights for practices, and collaboratively identify actionable steps for improvement through coaching care teams in process improvement and driving outcomes. 

What You Will Do 

  • Act as a liaison between Vytalize and physician practices to provide on-site and remote assistance and facilitation to the clinicians, care teams and their associated practices to drive improvement in clinical quality and operational efficiency.
  • Review and interpret population health data to identify opportunities to improve patient outcomes, utilization patterns, and cost trends.
  • Conduct meetings with the practices on a regular basis to communicate status reports, key drivers of change, leading or lagging indicators and providing insights to providers and practices.
  • Implement evidence-based protocols and other improvement initiatives. 
  • Supports, encourages and works closely with practice staff, providers, their care teams, and medical unit leaders to implement process and quality improvement activities and workflows necessary for successful participation in value-based agreements. 
  • Collaborate with care teams to identify patients in need of additional services or interventions.
  • Engage with internal and external stakeholders, including providers, payers, and community organizations, to align strategies and share best practices.
  • Maintain knowledge of current Vytalize Health initiatives and contractual updates associated with assigned practices.
  • Engage with practices to ensure Vytalize’ s awareness of their practices’ current and planned operations and/or objectives for value-based care.
  • Build strong relationships with Vytalize’ s partner practices (MDs & office staff)
  • Ensure practices are achieving upon agreed-upon KPI’s monthly
  • Responsible for communicating new initiatives to practice staff
  • Responsible for training physicians and practice staff on population health tools and software
  • Assist practices when technical issues arise with population health tools and software
  • Educate practices on ACO and CMS rules and regulations 
  • Willingness to travel up to 30% of the time to visit practices in person within your geographical location

What will make you successful here 

  • Bachelor's degree in public health, healthcare administration, nursing, or a related field. Master’s degree preferred. 
  • Previous experience in healthcare, population health management, or health analytics preferred.
  • 1-2 years of Population Health experience
  • 3–5 years of clinical administration experience
  • Strong analytical and data interpretation skills.
  • Familiarity with electronic health records (EHR) systems and health information exchanges (HIE).
  • Ability to collaborate with diverse teams and stakeholders.
  • Strong presentation skills 
  • Excel experience preferred.
  • Ability to implement and explain effective training and development.
  • Previous experience with revenue cycle management and CMS MIPS report, a plus
  • Excellent verbal and written communication skills
  • Excellent organizational skills and attention to detail
  • Comfortable working independently without close management 
  • Entrepreneurial spirit, a sense of ownership and comfortable operating in ambiguity
  • Solution oriented with the ability to think strategically and creatively in decision-making.
  • Strong work ethic to exceed expectations and goals.
  • Coachable and able to take direction and feedback well yet being forward-thinking to challenge the status quo.
  • Ability to act with integrity, professionalism, and confidentiality.
  • Valid Driver License with no restrictions preferred. 
  • Reliable transportation preferred 
  • Proficient with Microsoft Office Suite or related software
