The Opportunity: EMRGE Re-Entry Case Manager Coordinator
Job Type: Full-time, On-Site
Pay: $50,000- $55,000k/ year

As Case Manager Coordinator , you’ll have a dynamic role working with clients currently incarcerated and being released from the Essex County Correctional facilities.The Essex Medication Re-Entry Grant Expansion Program (EMRGE) is a program for incarcerated and/or justice-involved individuals with opiate use disorder. The program helps participants overcome barriers such as employment assistance, recovery, community support and mental health services. As Case Manager Coordinator you will help with the client's transition back to their community, and follow-up with their goals and action plans 6 months post release. As Case Manager Coordinator, you will work closely with the Program Manager and a dynamic team of clinicians, and correctional staff to ensure that clients receive the care and support they need to be successful.
