Title: Pathway Home Program Supervisor Division: HOPICS / Housing Stabilization
FLSA: Non-Exempt/Full-Time Supervisor: Program Manager
Pay Range or Rate: $2,686.66 to $2,946.66 Semi Monthly Revised: 3.12.2024
Summary:
Under the direction and supervision of the Pathway Home Program Manager the Pathway Home Program Supervisor will be responsible for providing leadership, mentoring, and training to Support Services staff, monitors, program coordinator and pet coordinator. The Program Supervisor will also support the Program Manager with directly supervising subcontracted partnerships (if applicable). The Program Supervisor will be responsible for supporting the Program Manager in meeting the program and fiscal requirements (if applicable) as set forth in the contracts; maintaining community relationships with public and private organizations; and providing reports and documentation as necessary for the operation of the project. Program Supervisor's primary responsibilities will be to conduct daily tracking of supportive services activities, be responsible for day-to-day oversight of field operations and office operations, provide field-based support as needed and monitor follow-through of client linkages and referrals.
Essential Functions:
•The supervisor will maintain and analyze program operations within the prescribed budget in regard to the Pathway Home Program; organize and implement a culturally competent project; utilize the designated evidence based for model for service delivery, including curriculum implementation; and coordinate with community partners and stakeholders.
•Maintain an in-depth knowledge of Time Limited Subsidy, and DHS policies and procedures and implementing changes as needed to support the Program Manager with maintaining program operations within the prescribed budget for assigned programs and contracts.
•Organize and implement a culturally competent project that aligns with the designated evidence-based model for service delivery, including curriculum implementation.
•Provide short and long-range planning in collaboration with program leadership.
•Effectively monitor contract compliance and completion of contract goals/objectives; work collaboratively with agency management and provide (oral/written) reports as necessary; prepare project reports in accordance with funding requirements (if applicable).
•Supervise client files and ensure adherence to funder definitions and guidelines. Maintain files/records on client services in compliance with HIPAA, 42 CFR Part 2 and other funding requirements for auditing purposes.
•Convene and facilitate 1:1 supervision, Pathway Home staff meetings; coordinate and facilitate staff development and training.
•Participate in and attend all special project meetings and training as required.
•Implement and monitor SSG personnel policies and procedures for compliance with applicable state and federal regulations.
•Supervisor will supervise day-to-day activities of the case managers, monitors, and program coordinator.
•Oversee and provide direction to ensure that all case management staff assist clients at every state of the housing stabilization process and oversee housing navigators with supporting clients with getting documents ready for housing.
•Communicate effectively with the contract monitor and funder, partner agencies, fiscal and human resources personnel, management, staff, and clients.
•Maintain landlord relationships as needed.
•Oversee the monitors to support the overnight staffing for 24hrs client support.
•Coordinate with the housing, quality assurance, and fiscal departments to ensure documentation completeness and financial assistance disbursements.
•Efficiently manage and monitor data integrity and quality assurance of client and program milestones and service outcomes.
•Maintain appropriate boundaries; adhere to SSG's Code of Ethics and HOPICS' Core Values and represent the Agency in a professional manner at meetings and community events.
•Maintain and uphold Agency mission statement, values, policies, procedures, and principles.
•Attend all weekly meetings with DHS, Pathway Home meetings to ensure the program is in compliance with the contract and client need.
Secondary Functions:
•Work on special projects as assigned.
•Perform other duties as assigned by Associate Director and/or Deputy/Division Director.
Supervisory Responsibilities:
The Supervisor will supervise the following staff: Case Managers, Monitors, Program Coordinator, and other personnel as necessary. Supervisor reports to the Pathway Home Program Manager and Associate Director of Housing Stabilization.
Minimum Qualifications - Knowledge, Skills and Abilities Required:
Bachelor's degree in systems, Social Services or other administrative or business sector is required from an accredited or state approved college or university, with a minimum 1-2-year job related supervisory or leadership experience; If in recovery, a minimum of three (3) years of being drug and alcohol free is required. High Proficiency in HMIS, Microsoft Word, Excel, Access, and other database programs. Accurate and concise documentation. Neat and professional appearance. Attention to detail and punctuality required.
MANDATORY Qualifications:
Ability to work with clients from diverse cultural, ethnic, and socio-economic backgrounds. Ability to communicate effectively, both written and orally. Valid California Driver's License and proof of insurance is required. Verification of Employment Eligibility and Background Check is required. TB test required (Note: Results may not be more than (3) months prior to or (7) days after date of hire and renewed annually thereafter.) CPR and First Aid certification required within 30 days of employment with the company.
Environmental Conditions (Working Conditions):
May be exposed to highly aggressive clients. Must communicate with many sources including public paying agencies, courts and outside community agencies. Local automobile travel is required. Some evenings and weekends may be required. Sometimes noisy, loud, and disruptive clients.
Physical Requirements:
The Program Supervisor typically spends time sitting, standing, walking, driving, and carrying (max. 25 lbs.), listening, speaking.
Mental Requirements:
This position will require the individual to be able to handle any/all the following: constant distractions, interruptions, uncontrollable changes in priorities/work schedules: be able to process information, think and conceptualize.
