• Provides one-on-one services for adults and/or children with behavioral health disorders; providing training and support that promote recovery. Services are primarily provided in the community
• Conducts assessments of persons served, completes intakes and develops individual service plans
• Monitors the following community living areas: housing, entitlements, education, finances, social/recreational, family living, mental and physical health, legal assistance
• Provides and documents job mandated services to meet current program budgetary goals
• Links persons served to needed services and acts as an advocate as needed
• Creates, develops, and maintains linkages with other service providers and community resources that may be of assistance to persons served
• Manages all case records in accordance with licensing and contractual requirements using an Electronic Health Record
• Develops and maintains positive relationships with funding and regulatory agencies
• Flexible schedule

• Must possess a bachelor's degree in Psychology, Sociology, Social Work, Counseling, Rehabilitation Services, Special Education, Early Childhood Education, Secondary Education, Family and Consumer Sciences, or Human Growth and Development
• Experience in the provision of direct client services in a social service related field preferred
• Must have safe, reliable transportation and ability to transport persons serviced in personal vehicle within program service area
