GENERAL SUMMARY: # This position works closely with the interdisciplinary health care teams to provide nutrition services and medical nutrition therapy that are integrated and compatible with the patient#s medical program goals and objectives.# Primary responsibilities include developing and implementing nutrition services and medical nutrition therapy using established standards of care and practice protocols/guidelines; managing resources in an effective manner; participating in performance improvement efforts. PRINCIPAL DUTIES AND RESPONSIBILITIES: (The following duties and responsibilities are all essential job functions, as defined by the ADA, except those that begin with the word #May.#) Demonstrates the knowledge and skills necessary to provide care appropriate to the age of patients served.# This includes knowledge of the physical and psychological needs of patients served and the ability to respond appropriately to those needs. Conducts nutritional assessments for patients at nutritional risk Develops and implements an individualized plan for nutritional intervention in accordance with the patient#s medical program goals and objectives Utilizes assessment and evaluation techniques that consider the varied needs of age specific populations as well as cultural, religious and ethnic concerns. Provides appropriate documentation that summarizes the nutrition care plan in the patient#s medical record, including nutritional assessment, diagnosis plan, implementation and progress toward needs. Demonstrates accountability for the proper use of patient#s protected health information. Utilizes patient scheduling and monitors/maintains patient through electronic medical record.################################ Assures that care delivered to each patient is in accordance with the state practice act. Assesses educational needs and provides nutritional counseling for individuals and groups. Evaluates achievements of learning objectives by the patient and/or family.# Provides appropriate follow-up in accordance with the patient#s treatment goals. Serves as a resource to medical staff and allied health personnel on nutritional needs. Assists in the education of new staff, dietetic students and interns.# Plans learning experiences, teaches and provides feedback for performance. Participates in community projects as assigned. Conducts inservices and educational presentations to hospital department staff. Communicates with the medical team through interdisciplinary meetings, rounds and medical conferences. Partners with food service staff to assure conformance to diet prescriptions. Assists in developing and revising patient education materials, clinical forms and protocols. Identifies and collects performance improvement data, monitors outcomes and implements corrective actions.############################ Participates in departmental in interdisciplinary meetings, task forces and projects.
