Vista Community Clinic is Hiring a Care Coordinator Near Vista, CA

Overview

Responsibilities

Coordinate and supervise one or more programs, provide oversight of administration and implementation of project work plans. Responsible for the development and tracking of clinical patient education and outcomes. Provide guidance and assistance to Vista Community Clinic patients by helping them manage their health care. 

  • Help to improve health outcomes and reduce health disparities for Vista Community Clinic patients with and without chronic diseases
  • Provide oversight of administration and implementation of care coordination model
  • Oversee completion of program objectives and recommend changes in model as needed
  • Serve as liaison and assist with collaborative support activities with clinical and operations staff
  • Assist in the development of a wide range of materials such as educational curriculum, fliers and news release

Qualifications

Minimum

  • Associate's degree in related field with experience in health education or care coordination
  • Bilingual English/Spanish
  • Ability to work flexible hours, including evenings and weekends
  • Valid California driver's license, vehicle insurance and reliable transportation; clean driving record 
  • Certification/minimum two years’ education in chronic disease/health management

Preferred

  • Bachelor's degree in related field or associate's degree with two years care coordination or health education experience

Required Skills/Knowledge/Abilities

  • Ability to present information effectively both verbaly and in writing to individuals and groups from various cultural, ethnic and educational backgrounds
  • Basic knowledge of computers, internet use, MS Office and data entry skills
  • Strong customer service skills
  • Ability to work independently, take initiative and be proactive
  • Ability to analyze, prioritize and meet deadlines
  • Ability to plan and facilitate internal and community meetings
  • Ability to handle confidential materials and information in a professional manner

Pay Range

$19.50 - $22.00
