The HCBS Program Manager supervises care coordination personnel and acts as the liaison between care coordination and the Executive Director of Community Care. This individual will ensure that the performance of team members is evaluated on an ongoing basis through periodic case file reviews as needed, and coordinate training for new employees so that they accompany the team lead and more experienced staff within the first 6 months of employment. They will submit a monthly schedule of daily activities for the first month of employment for new SW team members to the Clinical Director.
