JOB SUMMARY

The registered nurse’s responsibilities include collecting client health data and working with the patient/family to set attainable goals. The nurse shall also develop and modify the patient care plan as needed in collaboration with the physician, health care team, patient, and patient’s family. Registered nurse must have strong assessment, triage, IV, Hospice and wound care skills. The nurse must be able to work closely with weekend on call manager to perform visits as assigned. Qualified candidate must be committed to providing outstanding care and possess the following qualities: detail oriented, honesty, flexibility, organized, open minded, be a team player, and proficient computer skills. Must be available to work Friday 4pm-Monday 8am for scheduled & PRN visits. Must cover primary on call coverage Friday through Monday 4pm-8am.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  1. Interviews the client and/or family to obtain a comprehensive history and to obtain data about client/patient's developmental status and behavior.
  2. Examines the client to obtain objective physical data.
  3. Reviews available clinical data from referral and lab results, etc. as part of database.
  4. Completes a physical and psychosocial assessment based on interview, examination and review of available clinical data.
  5. Documents to be completed in the home, timely and according to guidelines.
  6. Communicates client status and assessed needs to members of interdisciplinary team.
  7. Reassesses client status at established time frames and as indicated by the client’s status.
  8. Reports status and changes in status as appropriate.
  9. Assesses potential for development of crisis situations based on combination of subjective and objective cues.
  10. Develops an individualized approach to deliver nursing care to the individual client/patient and family.
  11. Collaborates with interdisciplinary team to develop an integrated plan of care.
  12. Initiates discharge planning as an integral part of the plan of care at the time of admission.
  13. Documents plan of care appropriately.
  14. Acknowledges, transcribes, coordinates and implements the medical diagnostic and therapeutic orders.
  15. Promotes client/patient's dignity during provision of care.
  16. Provides calm, supportive and therapeutic environment for clients/patients and their families.
  17. Assesses the client/patient's response to and understanding of care provided.
  18. Advocates and coordinates client care for optimal function and for positive outcomes.
  19. Triage phone calls and determines need for off hours visit
  20. Reviews and updates the plan of care in collaboration with the client/patient/family and other interdisciplinary team members.
  21. Anticipates client/patient's abilities and incorporates anticipated needs into plan of care.
  22. Completes and submits Weekend Hand off report by 8 am Monday.

Requirements

EDUCATION AND EXPERIENCE

  • Bachelor's degree required. Master's degree is preferred.
  • Current Licensed RN with at least 3 years' experience in home health is required.
  • CPR is required.

REQUIRED SKILLS AND ABILITIES

  • Ability to deliver patient care in a manner that is appropriate to patient’s age, physical ability and intellectual development. Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the patient population served.
  • Ability to express or exchange ideas by means of the spoken word. Primary functions include activities in which incumbent must convey detailed or important spoken instructions to patients, physicians, families and other employees accurately, loudly or quickly.
  • Ability to receive detailed information through oral communication and to make discriminations in sound.
  • Ability to plan and perform diversified duties requiring knowledge of nursing of the patient including age related changes, and the use of a wide range of procedures. Involves the exercise of judgment in the analysis of facts and conditions regarding individual’s problems, concerns or cultural factors to determine the plan of care and what actions should be taken, within the limits of standard practice.
  • Ability to perform job alone following standard practices and any of several procedures, but refer questionable cases when they arise out of standard practice.
  • Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and co-workers.
  • Ability to communicate effectively and establish a cooperative, collaborative working environment.
  • Ability to accept constructive feedback and initiate appropriate actions to correct situations.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • Job requires some physical activities including but not limited to; sitting or standing for extended periods of time, typing, reaching, standing, walking, kneeling, lifting, fingering, grasping, feeling, talking and hearing.
  • Job requires use of general office equipment including phones, copiers and computer equipment.
  • Job requires light physical work including the ability to exert up to 20 lbs. of force occasionally, 10 lbs. regularly.
  • Job requires visual acuity including the ability to see at or within arm’s reach as well as to operate a motor vehicle.
  • Job is subject to inside environmental conditions; low noise level.
