Description

Do you think you could take patients calls, assess their clinical needs and if needed able to go out to their home during the hours from 4pm to 8am? While the majority of calls can be handled over the phone there are times when the patient will need to see you. Flexibility is needed during orientation as Initial training occur on weekdays during agreed upon daytime hours.

This position could be ideal for the right person! Preferred minimum qualifications include one to two years of nursing or triage. We are willing to train! Must be able to work independently, have excellent communication, strong clinical and assessment skills. BSN preferred, will consider Associates RN with relevant experience. NH RN license required. JOB SUMMARY:

Responsible for providing after-hours and on-call nursing service. Responds to telephone requests for assistance by addressing the request by telephone or by making a visit that includes skilled assessment and interventions. Delivers care to patients and families with needs ranging from basic to complex utilizing the nursing process. Creates a therapeutic and caring environment for patient and families. Practices in a manner consistent with the Franklin VNA & Hospice Practice model; the ANA’s Social Policy Statement, Scope and Standards of Practice, and the Nursing Code of Ethics; and relevant specialty standards of practice.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

  • Responsible for providing after-hours and on-call nursing service.
  • Responds to telephone requests for assistance by addressing the request by telephone or by making a visit that includes skilled assessment and interventions.
  • Demonstrates competency in skills required by the agency for patient care.
  • Utilizes the patient’s plan of care to maintain continuity of patient care by communication with appropriate staff.
  • Answers all incoming calls in a manner appropriate to the nature of the call, completes admits, and makes follow-up visits on patients admitted the day prior; as necessary.
  • Handles referrals and inquiries for services, rapidly identifies the caller’s needs, and takes proper action to assure the caller’s needs are handled.
  • Maintains flow of communication between in-house staff, field staff and other departments, for coordination of activities and services
  • Conducts follow up with office by phone and fax machine to ensure quality of care and notifies Administrator and/or Clinical Manager
  • Communicates with the physician regarding the patient’s needs and reports changes in the patient’s condition; obtains physician orders as required.
  • Develops, prepares, and maintains patient records with accuracy, timeliness and according to policies.
  • Submits accurate documentation in timely manner.
  • Document data and assessment on appropriate form according to guidelines.
  • Perform other duties as assigned. 

Requirements

EDUCATION AND EXPERIENCE

  • Graduate of an accredited school of nursing with a Bachelor of Science in Nursing (BSN) or equivalent education.
  • Current Certification/Licensure as a Registered Nurse in the State of New Hampshire is required.
  • Minimum of 1 year of progressive relevant professional experience, preferably in a homecare setting.

SKILLS AND ABILITIES

  • Ability to deliver client care in a manner that is appropriate to patient’s age, physical ability and intellectual development. Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the client population served.
  • Ability to express or exchange ideas by means of the spoken word. Primary functions include activities in which incumbent must convey detailed or important spoken instructions to patients, physicians, families and other employees accurately, loudly or quickly
  • Ability to receive detailed information through oral communication and to make discriminations in sound.
  • Ability to plan and perform diversified duties requiring knowledge of nursing of the patient including age related changes, and the use of a wide range of procedures. Involves the exercise of judgment in the analysis of facts and conditions regarding individual’s problems, concerns or cultural factors to determine the plan of care and what actions should be taken, within the limits of standard practice.
  • Ability to perform job alone following standard practices and any of several procedures but refer questionable cases when they arise out of standard practice.
  • Ability to demonstrate courtesy, compassion and respect in relating to patients, families, visitors, physicians and co-workers.
  • Ability to work as a team and maintain positive working relationships with employees and healthcare professionals.
  • Ability to document patient care accurately according to nursing standards and plan of care.
  • Working knowledge of Microsoft Office software and other industry specific programs.
  • Must demonstrate ethical conduct and build trust with patients and family members.
  • Ability to follow safety guidelines as instructed by the agency.
  • Ability to wear protective clothing such as mask, glove, head cover, lead apron, safety glasses, safety shoes, etc.
  • Ability to travel required. Travel throughout central New Hampshire is required.
  • Current drivers’ license is required.
  • Proof of automobile insurance is required. 
  • Ability to pass a motor vehicle background screening is required.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • Job requires physical activities including but not limited to, standing for extended periods of time, bending, lifting, reaching, kneeling, pushing, pulling, crouching, crawling, fingering, grasping, feeling, talking and hearing. 
  • Job requires moderate physical work including the ability to exert up to 50 lbs. of force occasionally, 30 lbs. regularly.
  • Job requires close visual acuity including the ability to see at or within arm’s reach.
  • Job is subject to inside environmental conditions, low noise level.
