Come join the growing Visiting Docs organization and provide meaningful, high quality, evidence based care to patients residing in an assisted living facility and in our out-patient clinic.

Looking for a motivated, hardworking, and experienced physician with experience in family medicine.

Ideal candidate will have previous experience working with patients in their residences, in assisted living facilities and in a clinic enviornment.

Responsibilities include but are not limited to:

• Provide general primary care to facility and clinic patients.

• Diagnose, treat, and manage both acute and chronic conditions.

• Prescribe and refill medications as appropriate.

• Actively communicate with patients, families, other specialists and facility staff (case managers, social workers, med room).

• Order and interpret diagnostics tests such as imaging and lab work.

• Documents patients’ histories and treatments in EMR.

• See patients via telehealth, as needed.

Qualifications:

• Active unrestricted New York State license.

• Minimum 3 years of experience preferred.

• PECOS enrolled.

• DEA certified.

• BCLS.

• Knowledge of eClinicalworks preferred.

Full time and part time schedules with no evenings or weekends required.

On-call (phone) for your patients only.
