POSITION SUMMARY
The Assistant Lead Case Manager is responsible for assisting the day-to-day operations of the case management department including the timely submission of school enrollment packets, reunification packets, service plans and other assessments required for the compiling of a comprehensive case file found with supporting documentation maintained in physical files. The Assistant Lead Case Manager will maintain a small caseload and will assist other Case Managers as needed. The Assistant Lead Case Manager works closely with the Lead Case Manager, Program Directors, Assistant Program Directors and other related departments within the Program. The Assistant Lead Case manager is required to maintain a flexible, organized and efficient work schedule and is subject to work extended hours, weekends, and be on-call as needed.

ESSENTIAL FUNCTIONS:
  • Assist Lead Case Manager with cases assignment immediately upon youth’s arrival or the next business day
  • Must ensure that youth are properly enrolled into the program’s internal intake system.
  • Must perform audits of the program’s active case files and maintain appropriate documentation to ensure compliance with VisionQuest, State, and Federal guidelines
  • Assist in the development of corrective action plans or disciplinary memos to address performance issues of case management staff in collaboration with Program Director
  • Review all assessments, Family Reunification Packets and any other documentation or assessments for accuracy of information while ensuring proper spelling and grammar is utilized prior to case manager submission to any stakeholder
  • Serve as the main point of contact for approving initial placements, youth being transferred and for requests for information in coordination with Program Director
  • Prepare and submit case log reports to program staff, local funding source representatives and other stakeholders as approved by the Program Director. Maintain a daily updated list of the status for all cases to ensure that adequate progress is being made.
  • Ensure that travel arrangements are completed for program staff transporting youth for reunification or transfer purposes (may include booking airfare, hotel, and rental car)
  • Ensure that Case Management Significant Incident Reports (SIRs) are submitted within 24 hours upon notification of the incident and that additional reports are submitted to CPS and internal reporting mechanism as required.
  • Carry a small caseload for youth particularly those that are identified as being high profile or complex cases.
  • Conduct initial intake interview with the UC to build rapport, gather potential sponsorship information, and establish age for the purpose of policy compliance
  • Identify available options for UC within 48-72 hours of arrival and proceed with the required documentation for a safe reunification.
  • Ensure assessments are conducted in a timely manner, assessments conducted by CM are: Initial Intake Assessment, Individual Service Plan, Case Review, and UAC assessments.
  • Keep a daily and constant communication and conflict resolution between internal departments (clinical, medical, education, security, youth care) and external stakeholders (consulates, CFS, FFS, GDIT).
  • Ensure UC signs the Legal Service Provider list, Notice to Juvenile Aliens in DHS or HHS Federal Facilities, and VisionQuest’s intake forms.
  • Interview and keep ongoing communication with sponsors in the US and families in Country of Origin and provide case updates.
  • Request documentation to complete FRA to determine options available for youth and proceed with the required documentation to reunify youth with family in their home country or in the United States as deemed applicable.
  • Build communication and meet with UAC on a weekly basis to provide updates and check on their wellbeing status.
  • Ensure the provision of two weekly telephonic contacts with family of origin, primary caregiver and/or sponsor.
  • Document all actions taken and contacts with youth, sponsor, family and stakeholders in the form of progress notes (efforts) as required by VisionQuest, State and Federal contracts.
  • Carry out high priority situations such as emergency transfers to Staff Secure Shelters due to violent or aggressive behavior of UCs as well as Age Redetermination for adults
  • Assist supervisors with additional tasks when the shelter needs increased due to influx seasons
  • Coordinate with FIRRP and pro-bono attorneys for the timely provision of “Know Your Rights'' presentations. Coordinate ongoing legal appointments and court hearings.
  • Complete and submit reunification packets for initial review to Lead Case Manager or Designee (if applicable).
  • Submit completed reunification packet with appropriate referral made by Case Manager for the timely release of youth to designated GDIT Case Coordinator and FFS
  • Facilitate attorney to client contact as requested by youth.
  • Attend case staffing with funding source representative on a weekly basis
  • Maintain physical and web-based client files.
  • Ensure that Significant Incident Reports (SIRs) are submitted within 4 hours upon notification of the incident and that additional reports are submitted to state licensing and VisionQuest’s internal reporting mechanism as required.
  • Drive clients to facilitate program services, may include transportation to court appointments, attorney visits, other appointments as per contractual requirements as needed. May also include transporting youth within the United States if reunified.
  • Attend all organizational required training.
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practice are adhered to.
  • Attend training that will enhance professional growth in the area of case manager and documentation or other topics as deemed appropriate by the program director.
  • Travel as needed for training, conferences or to transport youth to destinations located within the U.S.
  • Maintain a safe, clean and hazard-free work area.
  • Ensure the proper supervision of youth at all times.
  • Able to react to change productively and handle other essential tasks as assigned.

QUALIFICATIONS:
  • Bachelor’s degree in the behavioral sciences, human services or social services fields. At least 2 years of Child welfare and/or case management experience.
  • Must have excellent skills in verbal and written communication, excellent organization and time management skills, and familiarity and competency with personal computer applications.
  • Must be computer literate with working knowledge of Microsoft Windows (Word Perfect, Microsoft Word, Excel)
  • Must be able to obtain and maintain First Aid Certification (CPR) and fingerprint clearance
  • Must possess a valid state driver’s license and be eligible to drive to facilitate program services as required by contractual agreement.
  • Must be at least 21 years of age at the time of hire
  • Must pass a drug test and physical and must clear Tuberculosis test
  • Must be Bilingual in English and Spanish

WORKING CONDITIONS/PHYSICAL REQUIREMENTS:
  • May lift and carry up to 10 pounds frequently, up to 20 pounds occasionally, up to 50 pounds infrequently.
  • Performs tasks that may involve standing or walking 50% of the day; sitting 50%of the day.
  • May bend, stoop, reach and grasp as necessary.
The pay range for this role is:
64,608 - 64,608 USD per year (888 Steven Rogers Village)
